Instruments are instrumental

Every surgeon will tell you of times when the instruments they use every week can't be found.  One solution is to have lots of large sets.  But this comes at a huge cost; counting in and out, acquisition, sterilisation, storage etc.  It's often a massive problem in orthopaedics where sets and implants need to be aligned.  Working with theatre teams and suppliers to produce integrated solutions that everyone involved understands and are as lean as possible could make a huge difference to preparation and training time.  Some are already doing this.  We could do more.
