Idea from Twitter - Capacity

The best, most timely clinical decisions in the world can't get people home if they need social care support or placement and none are available. Stranded patients feel like biggest group by far.

I don't have complete view on the whole system even where I work (only the community, the front door in ED, and the very back door of the community hospitals) - from my viewpoint it's very much lack of social care capacity.
